Employment Forms
Every time you hire a new employee in Montana you are required to have them fill out these four forms.

Form W-4: Employee withholding allowance. This form determines how much federal withholding will be withheld from each paycheck. This form should be retained by the employer in the employees personnel file.

Montana Form MW-4: Montana Employee's Withholding Allowance and Exemption Certificate. This form determines how much Montana withholding will be withheld from each paycheck. This form should be retained by the employer in the employees personnel file.

Form I-9: Employment Eligibility Verification. This form is used to verify that any US citizen or noncitizen if authorized to work in the United States. This form should be retained by the employer in the employees personnel file.

Montana New Hire Reporting Form. This form is used to help the State of Montana locate employee's who owe child support. This form is to be filled out and sent into the State of Montana. The employer should also retain a copy in the employees personnel file.
